• Proof of residency (one of the following)
• Property Tax Payment Receipts
• Auto Registration • Lease/Rental Agreement • Mortgage Statement • Auto Insurance Statement • Voter Registration • Bank Statement • Health Insurance Statement • Pay Stub • State or Federal Income Tax Documents • Utility Service • Correspondence from a government agency
